Root cause analysis

Our team performs root cause analysis for clients around the world with the objective to understand the reason why accidents took place. Our team also performs failure prediction and reliability for complex systems such as power plants, cement plants, and oil and gas exploration and production.

Besides, our team also assists law firms that need international experts to perform root cause analysis for failures and accidents involving:

  • Explosions
  • Fire
  • Heat transfer
  • Structural deficiencies or materials failure provoking power plants failure

Supporting law firms in cases involving large accidents

This diagram shows an example of the crosslink between our capabilities and the areas that law firms in South America benefit from having us helping them.

Root cause analysis of fire fighting system for large buildings

Our team has also worked on root cause analysis of fire fighting systems for tall buildings. In one case for example, our team analyzed the Diesel Engine/Pump (Pumping System) that is installed in the basement of the building and the exhaust pipes connected to the Pumping System, and determined the root cause of unforeseen failures. Our team identified issues related to dynamic loads and thermal issues. Since our team has experts in Heat Transfer, Fluid Mechanics, Dynamics, and Internal Combustion Engines, that allows us to analyze from a multidisciplinary perspective.
